If you don t have a contract with them they don t have a leg to stand on. If I were you I would put my foot down and refuse to pay. If she threatens with legal actions then tell her you ll see her in court. That will either call her bluff or get something resolved through the courts.
Tell her you cannot accept liability for payment of the ad as it was placed without your approval and that the sales rep needs to pay for it. if they want to take you to court they are welcome to do so cause they have no contract or signed approval.
Tell her you refuse to pay because the ad was placed without your consent, and if she wants to sue you go ahead. If they have nothing from you giving them the OK to place the ad they ll back off.

You have nicely described the situation. And your description itself is answer to the solution.If they claim that, you have received advantage of that advertisement, then ask them to prove that. And, tell them that you have policy of making payments to contractors only if they have valid purcahse order or service order duly signed by you or any of your responsible employee. And, just claim that we have a system and just cant break the system because you overlooked our approval.I hope, your perfect professional answer wont allow them to come back again asking for payments.Even if the sales manager asks for some payment, tell her that, if she gets a valid purchase order, you will make her full payment, but not a single penny, if she has no supportings.
